What is Monash Children's at Home?

Monash Children's at Home provides in-home and community based nursing services for infants or children with ongoing, complex medical needs.

Goals

  • To facilitate safe and effective home-based care.
  • To promote parental confidence and competence in caring for their child at home.
  • To encourage continuity of care between the hospital and the community.
  • Liaise with the Medical/Nursing/Allied Health staff caring for infants/children during their hospital admission.
  • Liaise with community based health professionals such as GPs and Maternal and Child Health Nurses to ensure continuity of care following discharge from Monash Children's at Home.
  • To compliment, rather than duplicate, existing nursing services.
  • To shorten the length of hospitalised time, and enhance recovery time in a home environment.
