About Care in Context
Care in Context (CiC) is a HARP funded care co-ordination outreach service for children (0 – 18years). The focus of CiC is on continuing to improve the management of children with chronic diseases and / or complex psychosocial needs who frequently use Southern Health hospitals or are at risk of hospitalisation. Therefore decreasing inappropriate presentations to emergency departments and reducing the length of stay when an inpatient, whilst improving the child and families health and wellbeing. This is achieved utilising a family centred, holistic approach whilst incorporating a multidisciplinary team approach. A single health professional (care coordinator) takes a lead role in assisting families to:
- navigate the health system (helping to link people into appropriate services and supports)
- better manage health (social, emotional, physical needs)
- improve and coordinate communication between the family and clinicians
- develop a Care Plan
Care Co coordinators are based at Dandenong Hospital and Monash Medical Centre and local Community Health Services.
Cost – this service is free to clients
Referrals
Referrals are accepted from any health care professional or the family. Referrals are made via Southern Health Access. You can phone (03) 9554 9888 (1300 3icare) or contact a Mobile Access and Services Team (MAST) staff member to discuss your referral on -
