Social Work
Description of ServiceThe Social Work Department assists patients and their families with psychosocial support and advocacy through assessment, counselling, crisis intervention, discharge planning, information provision and accessing community services.Social Workers undertake a range of psychosocial interventions and implement therapeutic strategies with clients to achieve optimal patient outcomes. Children and families can be faced with a range of issues directly related to their hospital admission, and/or related to external factors which may impact on their long term safety and well being. Criteria for referralPatients can be referred to social work where social complexity exists and/or there is evidence of potential risk factors that may impact on a patient’s safety and well being.Hours of Service: Monday to Friday – 8.30am to 5.00pm After HoursThe After Hours Social Work Service is provided for:
