How to make a referral
Please refer as early as possible prior to the child’s anticipated discharge date. Contact any Monash Children's at Home staff member by phone, pager, fax or in person, and complete the relevant referral form (Paediatric/Neonatal). After a referral is made, the family is contacted and an assessment made as to the most appropriate service. CostThere is no charge for services to patients being discharged from Monash Children's.Families requiring long-term consumables will be invoiced. Services for overseas visitors and private patients can be provided on a 'fee for service' basis. |
