About Care in Context

Care in Context (CiC) is a HARP funded care co-ordination outreach service for children (0 – 18years).  The focus of CiC is on improving the management of children with chronic diseases and / or complex psychosocial needs, who frequently use Southern Health hospitals or are at risk of hospitalisation. Our aim is to improve the child and families health and wellbeing. A single health professional (care coordinator) takes a lead role in assisting families to:

  • navigate the health system (helping to link people into appropriate services and supports)
  • better manage health (social, emotional, physical needs)
  • improve and coordinate communication between the family and clinicians
  • develop a Care Plan

Cost – this service is free to clients
