About ChAT
The Children’s Asthma Team (ChAT) is a HARP (Hospital Admission Risk Program) funded asthma education program for children and their families. This program services the wider Southern Health Community with educators based across 3 community health services - Cranbourne, Dandenong and Parkdale. The ChAT educates families about asthma. This session is an overview which covers what is asthma, triggers, recognising symptoms, medications, drug delivery and action plans (including first aid).During your hospital stay you will receive basic information to allow you to go home and safely manage your child’s asthma. You will then be encouraged to follow up with your local doctor in 2 or 3 days, where you should go over an updated Action Plan. The Children’s Asthma team should then contact you within 10 days for a follow up FREE asthma information session. Parents find this session very helpful.
Self referrals are also accepted.
