If you child has had recent testing such as laboratory results, radiology films/reports or clinical history relating to your child’s diagnosis, please provide this documentation, particularly x-ray films, for the paediatric surgeon to review before or at the time of surgical consultation. This documentation is vital to your child’s treatment. Your child's records may be obtained from your primary care physician’s office or from the medical institution where the records originated. Our department accepts records by postal mail, fax, or hand-delivery.During your visit
This may be your child’s first time to visit the Monash Children’s Hospital; please provide the following items upon appointment check-in:- Concession card
- Information about closest relative (for example, name, address, phone number
- Insurance card
- Referral or order sheet from primary care physician
- Medical records, lab work, and x-rays
- Parking ticket for validation
- Advanced Directives or Power of Attorney (If you have these, please them bring with you. You may obtain a blank copy by contacting our Admitting Department
