Before your visitIf you child has had recent testing such as laboratory results, radiology films/reports or clinical history relating to your child’s diagnosis, please provide this documentation, particularly x-ray films, for the paediatric surgeon to review before or at the time of surgical consultation. This documentation is vital to your child’s treatment. Your child's records may be obtained from your primary care physician’s office or from the medical institution where the records originated. Our department accepts records by postal mail, fax, or hand-delivery.During your visitThis may be your child’s first time to visit the Monash Children’s Hospital; please provide the following items upon appointment check-in:
